NDIS Worker Screening check Drivers License and own transport Position Summary : The Support Worker delivers high-quality, person-centred care to clients living with disability, ageing needs, or complex health conditions. This hands-on role provides personal care, domestic assistance, community access, health monitoring, and goal-focused support in accordance with individual Care Plans. Support Workers are essential frontline representatives of Here To Help Home Care, ensuring safety, dignity, independence, and wellbeing for every client. The role requires professionalism, reliability, compassion, and adherence to regulatory and organisational standards, including NDIS, WHS, and SCHADS Award requirements. Key Requirements : 1-2 years’ experience in a similar role would be beneficial Experience working in aged care, disability support, community services, or a similar client-facing care role (preferred). Experience supporting clients with personal care needs, including mobility assistance and personal hygiene tasks. Experience writing progress notes, completing shift documentation, or recording client supports. Experience working with people with diverse needs, including dementia, mental health challenges, physical disabilities, or complex behaviours (preferred). Strong communication skills (verbal and written). Ability to follow care plans, instructions, and safety procedures. Time management and ability to work autonomously. Job Responsibility : During the course of your duties, work may be performed in private homes, community settings, or aged care facilities. Exposure to pets, varying home environments, and people with complex needs may occur. Personal Care Assist with showering, dressing, toileting, grooming, continence management, and personal hygiene. Support mobility, transfers, and use of assistive equipment in line with manual handling protocols. Maintain clients’ privacy, dignity, and independence at all times. Domestic Assistance Perform light household duties such as laundry, cleaning, dishwashing, vacuuming, and tidying. Prepare and cook meals as per the client’s preferences, dietary requirements, and care plan. Maintain a safe, hygienic home environment. Medication & Health Support Prompt, supervise, or assist with medication administration, strictly following documented instructions from a health professional. Monitor and report changes in physical, emotional, or behavioural health immediately. Apply basic first aid if required and escalate emergencies per policy. Community Access & Social Support Support clients to access appointments, shopping, recreational activities, education programs, and social groups. Encourage independence, skill-building, social participation, and meaningful routine activities. Facilitate community inclusion and reduce social isolation. Goal Support & Client Outcomes Work with participants to progress toward identified goals (e.g., independence, mobility, confidence, daily living skills). Apply strength-based, person-centred support practices. Documentation & Reporting Complete shift reports, progress notes, incident reports, hazard notifications, and electronic sign-in/out accurately and on time. Record supports delivered, client progress, concerns, and observations in line with organisational and regulatory requirements. Maintain confidentiality and comply with privacy legislation. Professional Practice & Compliance Follow all client-specific instructions in the Care Plan, including any complex support requirements. Work safely at all times and comply with WHS, infection control, manual handling, and incident reporting procedures. Maintain professional boundaries and ethical practice. As a Support Worker your role will be varied depending on the clients' particular needs and requirements but may include any of the following: Assistance with Daily Life Assistance with Daily Life This support category relates to assisting with or supervising personal tasks of daily life to enable the participant to live as autonomously as possible. These supports are provided individually to participants and can be provided in a range of environments, including the participant’s own home. Daily Personal Activities Assistance with Self Care Activities These support items provide a participant with assistance with, or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible. Assistance with Self Care Activities Night-Time Sleepover Support This support item provides a participant with assistance with, or supervision of, personal tasks of daily life where overnight support is needed, but the caregiver can sleep when not required to provide support. This support applies to any day of the week and on public holidays. This support item includes up to two hours of active supports provided to the participant for the duration of the period. Assistance from Live-in Carer This support item provides for a person who lives in the house of, or travels with the participant and provides assistance with, and/or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible. Assistance with Personal Domestic Activities This support item assists a participant to undertake or develop skills to maintain their home environment where the participant owns their own home or has sole or substantial responsibility for its maintenance. Includes assisting participant to do basic house and yard work. On-Call Overnight Monitoring This support item provides for overnight on-call assistance (either onsite or off-site) with, or supervision of, personal tasks of daily living. It applies to any day of the week and on public holidays. This support item is for an eight-hour period and includes up to one hour of active support provided to the participant for the duration of the period. High Intensity Daily Personal Activities Assistance with Self Care Activities These support items provide a participant with a need for High Intensity Supports with assistance with, or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible. Specialised Home-Based Assistance for a Child This support item provides specialist assistance in the home that is required due to additional requirements of a child’s disability and may be provided to strengthen the sustainability of informal supports. Assistance with Household Tasks These support items enable participants to maintain their home environment. This may involve undertaking essential household tasks that the participant is not able to undertake. Assistance to Access Community, Social and Recreational Activities Participation in Community, Social and Civic Activities These support items enable a participant to engage in community, social and recreational activities. Participation in Community, Social and Civic Activities – High Intensity Supports These support items enable a participant who requires High Intensity Supports, to engage in community, social and recreational activities in circumstances where a more skilled or experienced support worker is required.
