This hands-on role provides personal care, domestic assistance, community access, health monitoring, and goal-focused support in accordance with individual Care Plans. Support Workers are essential frontline representatives of Here To Help Home Care, ensuring safety, dignity, independence, and wellbeing for every consumer. Key Requirements : Experience & Skills Industry Experience: 1–2 years in Aged Care, Disability, or Community Support (preferred). Technical Care: Proven experience in personal care (hygiene, mobility) and supporting clients with complex needs (Dementia, Mental Health, or physical disabilities). Communication & Admin: Strong verbal and written communication skills, including the ability to write accurate progress notes and follow detailed Care Plans. Physical Fitness: Capability to safely perform manual handling tasks, including standing, bending, and assisting with client transfers. Job Responsibility : We are seeking compassionate and dedicated Support Workers to provide high-quality, person-centred care to consumers, in their homes, community settings, and aged care facilities. This varied role focuses on empowering consumers to live autonomously and achieve their personal goals. Key Responsibilities Personal Care: Assist with hygiene, grooming, dressing, and mobility/transfers using safe manual handling protocols. Daily Living & Domestic Support: Perform light household duties (cleaning, laundry) and prepare meals according to dietary requirements. Health & Medication: Supervise medication administration, monitor physical/emotional wellbeing, and apply basic first aid when required. Community Engagement: Facilitate social inclusion by supporting consumers with appointments, shopping, and recreational activities. Complex & Specialist Support: Provide high-intensity care, overnight sleepover support, or specialised assistance for children as required. Documentation: Maintain accurate shift reports and progress notes in compliance with privacy legislation.
