About the business Hospital in Your Home (HIYH) is an innovative, patient-focused team that aims to provide the best possible care for our patients in the comfort of their own homes as an alternative to acute in-hospital care and Palliative Care. We offer 24-hour access to a nurse (in person or via telehealth) and care 7 days a week. About the role The position involves providing treatment for acute, palliative care and older patients in their homes, who would otherwise require hospitalization. The tasks include administering IV antibiotics, managing wounds, conducting complex assessments, troubleshooting, providing palliative end-of-life care, administering day infusions, and developing care plans, all while enjoying the countryside. What to expect: Exposure to a variety of hospital settings and working with like-minded Hospital in Your Home's exceptional healthcare clinicians Work within an innovative, supportive team work environment There may be an on-call component at times required, for appropriate candidates. You may be required to attend shifts at short notice, or up to six weeks in advanced if available. You must be available to work a minimum of one weekend per fortnight You will be part of on on call roster for over the phone care and referral management after hours. You are an independent practitioner who utilises excellent clinical skills, advanced critical thinking and communication skills. Through your hard working and enthusiastic nature, you will have the ability to adapt to changing settings and work shifts that suit your lifestyle. What you need to have: Minimum 1 years post graduate experience in acute nursing, with current continuum of practice Current IV cannulation certification preferred or a willingness to complete soon after commencement Critical Care qualifications or experience, is desirable Complex Wound Care skills is desirable Understanding of Community based services is desirable Current AHPRA registration - unendorsed Current Queensland drivers license Relevant working visa Ability to pass a police check Palliative care experience is also desired but not mandatory
