Purpose of the Role The Specialist Support Coordinator is responsible for providing high-level coordination for participants with complex needs , including psychosocial disabilities, behaviours of concern, and multiple service involvement. This role focuses on: Building participant capacity Reducing service fragmentation Managing complex risks Ensuring effective implementation of NDIS plans K ey Responsibilities Deliver Specialist Support Coordination in line with NDIS guidelines Support participants to understand and implement their NDIS plans Develop and maintain service agreements and support arrangements Build participant capacity to manage supports independently Complex Case Management Work with participants with: Psychosocial disabilities (e.g. schizophrenia, BPD) Forensic or justice system involvement High-risk behaviours or complex needs Identify, assess, and respond to risks including: Safeguarding concerns Service breakdown Environmental or behavioural risks Service Coordination Coordinate multidisciplinary teams including: Allied health professionals Community and mental health services Justice and forensic services Ensure participants are linked to appropriate supports and services Monitor service delivery and outcomes Documentation & Compliance Maintain accurate, timely, and professional documentation: Case notes (aligned with NDIS standards) Progress reports Incident reports Risk assessments Ensure compliance with: NDIS Practice Standards NDIS Code of Conduct Organisational policies Advocacy & Capacity Building Advocate for participants' rights, choice, and control Support decision-making and independence Empower participants to access mainstream and community services Key Skills & Competencies Strong understanding of NDIS Support Coordination framework Ability to work with high-risk and complex participants Excellent communication, negotiation, and problem-solving skills Strong documentation and reporting capability Ability to manage conflict, crisis, and high-pressure situations Cultural competence and trauma-informed practice Mandatory Requirements Relevant qualification (e.g. Social Work, Psychology, Community Services, Mental Health) Minimum 2–3 years experience in disability, mental health, or support coordination Knowledge of NDIS legislation and Practice Standards NDIS Worker Screening Check Working With Children Check Current First Aid & CPR Valid driver’s licence and access to vehicle ⭐ Desirable Requirements Experience working with: Forensic / justice system participants Complex behaviours and restrictive practices Knowledge of: Behaviour Support Plans Mental health frameworks Crisis intervention models Work Environment Community-based role with travel Flexible working conditions Collaboration with internal teams and external stakeholders Why Work with Optimum Care Plus Opportunity to work with complex, high-impact cases Strong organisational support and leadership Growth and development opportunities Values-driven, person-centred organisation Job Type: Casual Pay: $40.00 – $50.78 per hour Work Location: In person
