Your opportunity As a Care Partner, you’ll be a trusted advisor and advocate for clients, helping them navigate services and funding to remain living safely and independently at home. You’ll make an impact by Developing and adapting personalised care plans that reflect each client’s goals, needs, and aspirations Proactively identifying and responding to changing client needs in partnership with families and multidisciplinary teams Delivering high-quality, client-centred care that supports positive outcomes and experiences Managing client feedback and maintaining clear, timely communication with stakeholders Supporting operational efficiency through accurate administration and coordination tasks What you’ll bring To succeed in this role, you’ll need the following qualifications: Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields Experience in client case management National Police check Driver’s licence We would also love you to bring the following skills: Genuine compassion and empathy, with the ability to connect with people from diverse backgrounds and understand their individual needs Strong relationship-building skills, creating trust and rapport with clients, families, and colleagues A collaborative mindset, working effectively within teams while empowering others Adaptability and strong organisational skills to manage changing priorities with confidence A proactive, solutions-focused approach with the energy and enthusiasm to continuously improve and innovate Why join ECH?
