The Role We are looking for an experienced Care Partner to join our Community Health and Care team in Gisborne. This role suits someone who is organised, adaptable and comfortable managing a dynamic caseload within a fast-moving community care environment, balancing care planning, service coordination, documentation and client engagement. With a focus on Safe, Respectful and Person-Centred Care Management Skilled at service planning, monitoring and coordination Able to deliver older person advocacy and engagement Demonstrating an ability to ensure compliance and continuous improvement practices With attention to information management and confidentiality Places an importance on teamwork and professional practices About you To be successful in this role you will need: A qualification of cert III or higher in a health related field, aging or community Person centered practice Experience in care planning and budget development An understanding of service coordination Effective at managing a case load An understanding of Support at Home funding and guidelines All persons at Benetas will need to obtain a NDIS Worker Screening Clearance to be eligible for employment or engagement.
