This Clinical Nurse role provides advanced clinical leadership, patient-centred care coordination and education within a multidisciplinary team dedicated to improving outcomes for patients experiencing dementia, delirium and cognitive impairment. About This Role Your new role: As a Clinical Nurse within the Dementia and Delirium Nursing Service, you will deliver advanced specialist nursing care for patients experiencing complex cognitive, behavioural and psychological symptoms associated with dementia and delirium. Working across inpatient units, you will provide expert clinical consultation, lead person-centred care planning and support multidisciplinary teams to deliver safe, compassionate and evidence-based care. This role offers the opportunity to practice with a high level of autonomy while contributing to education, research and quality improvement within a specialised service. Key Responsibilities Include Provide advanced clinical assessment and management of patients with dementia, delirium and cognitive impairment Develop and implement individualised care plans and behavioural support strategies Deliver specialist consultation and support to multidisciplinary teams across inpatient units Provide leadership, mentorship and supervision to nursing staff and students About You You are an experienced Registered Nurse with a strong clinical interest in dementia and delirium care, capable of working autonomously while leading and supporting multidisciplinary teams. You bring advanced assessment skills, excellent communication and a commitment to improving outcomes for vulnerable patients with complex cognitive needs. About Us The Dementia Delirium Unit in Princess Alexandra Hospital delivers quality, coordinated interprofessional care optimising the health outcomes for people who present, or are at risk of presenting, to hospital with a range of complex symptoms and vulnerabilities associated with dementia and delirium, including: Patients with complex syndromes, symptoms and vulnerability related to dementia, delirium or cognitive impairment referred from all wards and departments of the hospital by either treating teams, nursing staff and/or allied health. Patients who are specialled for behavioural and psychological symptoms and/or falls related to cognitive impairment. Patients in Cognition Support Unit (5A CSU), a 10-bed specialised care unit under a Geriatrician Patients in Acute Cognitive Unit (ACU), an eight-bed specialised secure unit for severe BPSD Patients referred by GP in the community for Nurse Navigator Support when the patient presents high risk of imminent hospitalisation (as per PAH Nurse navigator catchment area based on postcodes).
