The Opportunity As a Care Partner within our Carrington at Home service, you will guide older people through their care journey with compassion and expertise – across the Support at Home (SaH) program and the Commonwealth Home Support Program (CHSP) - supporting people to stay safe and independent at home. Enjoy this varied, full-time role working onsite 8:00am - 4:00pm, Monday to Friday within our supportive, multidisciplinary team. Responsibilities: Delivering high standard case management & service coordination that is informative and responsive to individual goals and choices. Collaborating with participants and representatives to deliver an individual goal plan that supports assessed needs, goals and package resources. Working with the Clinical Care Partner to coordinate clinical care delivery within the quality framework. Ensuring that service delivery is within participant allocated service budget & managing unspent fund balances for the participant. Monitoring service outcomes, ensuring continuous improvement and client satisfaction, and that key KPIs are met Supporting all participants with navigating My Aged Care and aged care services as required. Participating in an on-call roster (remotely) to ensure timely service provision. About You Essential Criteria: Qualification in Aged Care, Community Services, or a related field; (or equivalent work experience). Experience and knowledge of My Aged Care, Support At Home (SaH), Home Care Packages (HCP) and/or Commonwealth Home Support (CHSP) Programs. Demonstrated experience in coordinating care services, managing budgets, and working with diverse clients. Have an understanding of the Strengthened Standards, Aged Care Act and related regulatory frameworks. Excellent communication and interpersonal skills, with a compassionate and client-focused approach. Driver’s License & First Aid Certificate. Unrestricted right to work in Australia and ability to obtain a background check (National Criminal History Check / NDIS Worker Check). Completion of a pre-employment assessment during the recruitment process.
