About the Opportunity The Care Partner Support Officer (CPSO) plays a vital role within the Care Partner team, providing comprehensive administrative and coordination support while managing a client caseload under the Wellness Model of Care. Acting as a primary point of contact, the CPSO delivers exceptional customer service and facilitates effective communication between clients, advocates and the wider care team. The role involves monitoring client wellbeing, identifying and escalating concerns to Senior Care Partners when required, and ensuring accurate, timely documentation is maintained across relevant systems. Through strong organisation, attention to detail and collaboration, the CPSO supports continuity of care and contributes to positive outcomes and experiences for our customers. About You You are a committed and compassionate professional who is sensitive to the diverse needs of older people from a range of social, economic and cultural backgrounds. You excel at building rapport with clients and their families and bring kindness, enthusiasm and energy to your work. You enjoy empowering others, working collaboratively with colleagues and balancing competing priorities in a fast-paced environment. You are detail-oriented, with strong administrative and time management skills, and can confidently manage multiple tasks while maintaining accuracy and meeting required timeframes.
