Support at Home Care Partner - Open to Cert III through to RN experience Experience working in Home Care as a Care Partner, case manager or client coordinator Permanent PART TIME - 32 hours per week (working over 5 days) Hours are flexible through the day approx 10am - 4.30pm Competitive rates dependent on experience plus super Salary packaging Available - $18,550 The Role: Oversee a caseload within the community, completing thorough clinical assessments. Create and deliver tailored care plans that reflect client goals and funding requirements. Deliver clinical services such as wound management, medication administration, and support for chronic conditions. Supervise and guide a team of Support Workers to ensure safe, high-quality care outcomes. Work in partnership with clients, their families, allied health providers, and community organisations. Track budgets and maintain adherence to Aged Care Quality Standards and internal policies. The successful candidate will have: Open to seeing candidates with Cert III through to RN AHPRA experience Experience as a and Care Partner, Case manager or Client coordinator Strong knowledge of Home Care Packages and Aged Care Quality Standards. Excellent communication and organisational skills. Current driver's licence with access to a car. National Police Clearance and Flu vaccination (no older than 6 months).
