About the Role This position focuses on facilitating community developmentinitiatives and supporting individuals to address issues relatedto recreation, health, housing, employment, and overallwellbeing. The role involves working collaboratively with clients,families, and community services to achieve meaningfuloutcomes. Key Responsibilities Facilitate community development initiatives and supportcollective solutions to community issues Maintain strict confidentiality of sensitive information Build positive relationships with participants and their families Monitor, assess, and report on client progress Identify community needs and available resources related tohealth, welfare, housing, employment, and training Support participants in achieving their individual goals Assess client needs and develop, implement, and review careand support plans Refer clients to appropriate external agencies for additionalsupport Provide care, education, and support to families, children, andindividuals with disabilities Assist young people in managing social, emotional, andfinancial challenges Engage with clients to understand and assess their needs anddifficulties Provide assistance with daily living, personal care, home care,and domestic duties Desirable Qualifications Certificate III or IV in Disability Services, Aged Care, or a relatedfield Additional relevant qualifications How to Apply To apply, please submit your resume along with a cover letteroutlining how your skills and experience align with therequirements of this role.
