Key Responsibilities Build and maintain positive relationships with participants,families, carers, and healthcare professionals from diversebackgrounds Deliver high-quality, person-centred, and evidence-basednursing care Monitor, assess, and document participant health status andprogress Develop and implement individualised care plans incollaboration with participants and stakeholders Provide supervision, guidance, and support to support workersand junior staff Work collaboratively within a multidisciplinary team to achieveoptimal outcomes Promote independence, dignity, and wellbeing of participants Participate in continuous improvement initiatives and maintainbest practice standards Ensure compliance with organisational policies, procedures, andrelevant healthcare standards To Be Successful in This Role, You Will Have: Current registration with AHPRA (essential) Previous experience in community care, disability services, or asimilar healthcare setting Strong clinical assessment and care planning skills Excellent communication and interpersonal abilities Ability to work independently and as part of a multidisciplinaryteam Flexibility to work across a variety of shifts, including weekendsif required Commitment to delivering high-quality, participant-centred care
