The Role The Team Leader, Home Care Queensland is responsible and accountable for the leadership, delivery and continuous improvement of high-quality in-home services for all Support at Home Packages as well as Veteran Home Care (VHC) funded services. This role is also responsible for managing a small case load of Support at Home clients. Reporting to the Manager, In Home Support, this position ensure that Elders receive care and support that is responsive to their needs, culturally appropriate, and aligned with best practice standards and aged care reforms. The position plays a critical role in driving operational performance, enabling business growth, supporting workforce development, and leading change management initiatives across the service. As a leader with MiCare, this role provides strong leadership in delivering on and upholding MiCare’s Mission, Vision and Values in alignment with the organisation’s strategic plan. What We’re Looking For Key Result Areas Ensure delivery of high-quality, person-centred care aligned with Support at Home and VHC program requirements Maintain accurate, up-to-date client documentation (care plans, reviews, progress notes) Lead, coach, and support SAH Care Partners and Veterans Home Care Coordinator Oversee intake processes for Support at Home, and VHC clients Monitor timeliness of onboarding, assessments, and care plan development Ensure accurate data entry and client profile creation in the client management system Oversee clients’ quarterly budgets to ensure SAH services do not exceed budget Qualifications Tertiary qualification in Social Work, Nursing, Aged Care, Disability, Allied Health, Business Management, or a related field. Current registrations as required i.e. AHPRA Registration. Qualification in management and leadership, or equivalent workplace experience would be highly regarded. Experience Demonstrated leadership experience in aged care, community services, or a related human services field. Demonstrated knowledge and sound case management experience. Clear understanding of the Aged Care Quality Standards A working understanding of health and safety requirements relevant to the position Solid knowledge of the Support at Home and Veterans Home Care program rules and guidelines. Leadership experience in community aged care, with empathy and sensitivity to the needs of older people including those from different cultural backgrounds and their families. Knowledge of consumer-directed care, wellness, reablement and person-centred service models. High-level skills in service planning, delivery oversight, and performance monitoring. Ability to manage change, implement reforms, and drive continuous improvement. Experience in budget oversight, risk management, and reporting. Excellent interpersonal, communication and stakeholder engagement skills. High level of proficiency in Microsoft Office and client management systems. Why MiCare?
