As a Dietitian with Empower, you will have the opportunity to work with a variety of patients within home care in the Western region of Melbourne. You will: Complete comprehensive nutritional assessments for residents and for clients in their own home through support at home packages/NDIS. Develop personalized nutrition plans and goals Support the dietary requirements of individuals with difficulty swallowing and chewing, bowel and intestinal issues and other complex issues. Provide weight management, poor appetite and dysphagia interventions Manage food intolerances and allergies Provide education on diabetes management Educate clients and their families to optimize their ability to live healthy lives within the community Work holistically with other allied health professionals to provide a multidisciplinary care plan Liaise with service providers to ensure effective communication Ensure caseload and documentation are timely and completed at a high standard You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.
