We are seeking an Assistant Program Facilitator to support clients with various disabilities through the provision of high-quality centre-based and/or community-based activities/programmes, that assist clients in achieving their goals while enhancing their quality of life. In this position, you will work across both of our service sites - Booval & Moorooka. Responsibilities include: Provides assistance to the Facilitator in developing and coordinating a range of centre based and/or community-based activities and programmes which reflect the individual needs, interests and aspirations of individual clients. Ensures clients are offered the opportunity and positively encouraged to make their own decisions and choices about their lives and lifestyles. Works with the Facilitator to implement effective risk management strategies to assess and minimise any risks associated with the delivery of services, in accordance with CPL policies and procedures. Maintains necessary records of service provision, including accurate reporting; and ensures relevant information is communicated to appropriate stakeholders. Assists the Facilitator to maintain effective relationships with clients and their representatives, advocates, local team members and other service providers involved in the implementation of clients’ individualised plans. Selection Criteria includes: Demonstrated experience in supporting and encouraging individuals with disability support needs to achieve their goals. Demonstrated experience in providing quality community-based programs and activities which support individuals with disability support needs, and the ability to develop and implement effective individualised client Programs of Support. Effective interpersonal and communication skills and the ability to negotiate and resolve conflict. Demonstrated experience in supporting individuals with personal care needs, including lifting and transfers. Demonstrated ability to work in both a team environment and independently in a variety of settings. Well-developed administrative and organisational skills including demonstrated intermediate skills in computer software applications (Microsoft Office suite of programs).
