Description The opportunity: We are seeking a Client and Carer Engagement Liaison to join our supportive, professional multidisciplinary teams. This is a flexible part‑time role (0.6 FTE) offered on a fixed term contract until 1 March 2027 . This role provides a warm, welcoming engagement experience for clients and carers, supporting intake, assessment, administration, and program participation . Responsibilities include but are not limited to; Coordinate client onboarding, including welcome calls, needs and risk assessment, and matching clients to suitable social activity groups. Respond to referrals for the Carers’ Support Program, providing phone-based holistic needs assessment, coordinate and connect carers to appropriate supports and manage invoice and reporting requirements for the Program. Contact clients after trial sessions, address feedback and support them in the next steps. Liaise with group facilitators to ensure a warm transition for new participants joining social activity groups. Document, manage and maintain accurate, up‑to‑date client notes and onboarding documentation in line and compliant with organisational systems and processes. Work closely with program coordinators, support workers, and intake and access teams to ensure smooth onboarding of clients, and services and supports for carers. Skills And Experiences Our ideal candidate: Excellent verbal and written communication skills and active listening skills. Friendly, approachable, client‑centred manner; able to build rapport with older adults and carers. Strong organisational skills with attention to detail and adherence to relevant program guidelines and timelines. Ability to identify potential risks or barriers to participation and respond appropriately using evidence informed approaches. Ability to engage with diverse stakeholders, internal and external. Competence in record‑keeping, basic digital systems, and working within budgetary requirements.
