Plan, deliver and evaluate individualised interventions to reduce pain, restore mobility, improve balance/coordination, and build capacity. Provide education, prevention and self-management strategies for clients, carers and stakeholders. Maintain high-quality records and collaborate with internal and external partners to optimise outcomes. Contribute to service improvement and community health initiatives.
