About the role This position is responsible for delivering best practice support to a case load of Support Coordination participants and contributing to continuous improvement of the service. Using best practice approaches, manage and undertake a Support Coordination case load of NDIS funded participants (or their representative) and relevant service providers to ensure that individual needs are being met through packages and assist plans coming to fruition Engage, build relationships and negotiate with the public sector and community based service systems for delivery of appropriate supports in accordance with NDIS plans Contribute to planning and implementing best practice systems and procedures for the operation of NDIS services Maintain required information within set timeframes including processes to ensure accuracy of data Support and educate individuals and/or their representatives on their rights and responsibilities concerning their funded plans About You The successful candidate should have: Minimum 3 years experience working in Community Services using the person-centred approach, including at least 2 years experience providing NDIS Support Coordination or similar community-sector roles. Working with people with mental health issues and those from a culturally and linguistically diverse background to meet their goals Current First Aid certificate Current driver’s license Intermediate IT skills including Microsoft suite of products and other applications Desirable Qualifications Minimum Diploma in Social Policy, Community Development, Disability, Community Services or relevant What’s on offer We are offering a 28-hour part-time term contract for up to 24 months, with an attractive remuneration from $87,851.88 to $96,771.28 plus 12% superannuation.
