This position will go into participants homes to engage them in all aspects of clinical assessments, coordinating referrals to allied health professionals, and working with the wider team to meet their cultural, emotional, physical, social and spiritual goals. About You To be successful in this role, you will be a Registered Nurse with current AHPRA registration, and at least 3 years’ experience as an RN. You will be customer focussed, take pride in your work, work as part of an effective team and deliver and support excellent care, which is person-led, safe, effective and connects us all. You will be able to: Demonstrate advanced level of clinical knowledge and expertise relevant to Community nursing services required to meet clinical support requirements, including the information and education required to support participants and carers to make decisions and take affirmative action in relation to their health needs. Strong cultural sensitivity with high cross-cultural communication ability Utilise an electronic documentation system and undertake documentation. Work autonomously while retaining the ability to work within a team setting. Effectively manage time and organise workload. Current full driver’s licence and ability to drive. Satisfactory completion of a National Police Records Check (arranged by Vasey RSL Care). Valid working rights which will be subject to a VEVO check.
