Job Description As our Customer Care Manager, you are passionate about making a meaningful difference in the lives of older Australians. We are looking for a proactive and compassionate Care Manager to join our dedicated team, supporting clients in the Melbourne Metro region. In this client-facing role, you will be the primary point of contact for new and existing home care clients, guiding them through their aged care journey with empathy and expertise. You’ll be responsible for delivering high-quality and personalised home care solutions, and developing strong relationships with clients and their support networks. Responsibilities include conducting assessments, developing and reviewing care plans that support independence and wellbeing. The position works closely with Registered Nurses, Allied Health professionals, and internal teams to coordinate services, ensure efficient service delivery, and match suitable care workers to client needs. Additionally, the role supports client onboarding, maintains regular communication to monitor satisfaction, and promotes services to referrers and stakeholders to grow the client base. The ideal candidate has strong communication and relationship-building skills with a customer-first mindset, along with experience in customer-focused service environments. They demonstrate a proactive, results-driven approach and the ability to meet targets and deadlines, supported by a solid understanding of the home care or broader health services sector. The role also requires confidence in engaging stakeholders and building external partnerships, the ability to work both independently and collaboratively within a team. For this role you will need a reliable car and driver's license, in return you will receive a petrol allowance.
