Employment Type: Permanent Full Time Position Classification: Registered Nurse Remuneration: $74,317.94 - $104,336.27 Hours Per Week: 38 Requisition ID: REQ644800 Applications Close: Sunday 22nd March 2026 About The Service The Kurri Kurri Transition Care Program (TACP) team forms part of the broader Hunter Valley TACP service, with colleagues also based in Muswellbrook and Singleton. The service provides short-term, goal-oriented support to older people following discharge from hospital. TACP is a 12-week enablement program designed to assist clients aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people) to regain independence and return to their pre-admission level of functioning where possible. Care is delivered by a multidisciplinary team including Registered Nurses, Assistants in Nursing, Enrolled Nurses, Physiotherapists, Occupational Therapists, Speech Pathologists, Social Workers, Dietitians and Domestic Assistants. The program emphasises a collaborative, person-centred approach focused on enabling and empowering clients to maintain independence and wellbeing. Role Overview The Registered Nurse / Case Manager is responsible for coordinating and managing client care throughout the twelve-week Transition Care Program. The role involves working collaboratively with the multidisciplinary team, clients, carers, families and external stakeholders to ensure coordinated care, appropriate referrals, and effective forward planning for clients following completion of the program. Practice is guided by the Aged Care Act 2025 and Transition Care Program Guidelines to support safe, effective and sustainable outcomes for clients post discharge. Key Responsibilities Provide case management and coordination of care for clients enrolled in the Transition Care Program. Undertake comprehensive assessment, care planning, monitoring and evaluation of client needs and progress. Work collaboratively with the multidisciplinary team to support client rehabilitation and functional recovery. Facilitate referrals and service linkages to ensure continuity of care following completion of the program. Maintain accurate, timely clinical documentation in accordance with service standards and legislative requirements. Communicate effectively with clients, carers, families and stakeholders to support shared decision-making and goal setting. Promote a person-centred, enabling approach that empowers clients and carers to maintain independence and wellbeing.
