With a strong presence in Western Sydney, the organisation supports the older Chinese community to live safely, independently, and with dignity in their own homes. Due to continued growth within their Home Care Package program, they are seeking an exceptional and compassionate Care Partner to join their dedicated Support at Home team. The Role Reporting to the Operations Manager, your main responsibilities will include: Coordinate and oversee services for clients receiving personalised support at home Conduct initial and ongoing client assessments, including risk assessments Develop, review, and update individualised care plans in partnership with clients and families Monitor client wellbeing and changing needs, ensuring services remain appropriate and responsive Manage Home Care Package budgets, ensuring transparency, sustainability, and alignment with client goals Liaise with rostering teams and care workers to ensure timely, safe, and effective service delivery Maintain accurate and compliant documentation in line with Aged Care Quality Standards Manage compliments, feedback, and complaints, including investigation, resolution, documentation, and escalation where required Support continuous quality improvement initiatives and compliance audits Build and maintain strong relationships with clients, families, and community stakeholders Your Experience Minimum 1-2 years' experience in a similar care coordination or case management role within Residential Aged Care or Home Care Mandarin or Cantonese language skills highly regarded, with a warm, culturally sensitive, and professional approach Relevant qualifications in Aged Care, Community Services, Care Management, or Nursing (e.g., Certificate IV in Aged Care and/or Home and Community Services) Demonstrated understanding of Aged Care Quality Standards, regulatory requirements, and compliance obligations Strong organisational and time management skills with the ability to manage competing priorities Experience working with culturally diverse communities (highly desirable) Proficiency in care management systems and Microsoft Office applications Excellent written and verbal communication skills If you are interested, please click APPLY and send through an up-to date CV.
