Anchor is seeking a skilled and compassionate Case Manager to join our MyLife team, delivering person-centred, trauma-informed support to children, young people, and families. This is a unique opportunity to work in a multi-disciplinary team delivering Targeted Care Packages – flexible, individualised support to help young people transition out of (or avoid entering) residential care. Your role as Case Manager You’ll work closely with Youth Workers and other key supports to: Deliver holistic case management tailored to the needs and goals of each young person. Partner with young people, families, and professionals in their care team. Embrace evidence-based practice grounded in developmental trauma, positive psychology and self-determination theory. Promote safety, stability, connection and personal growth for every client. Record and evaluate outcomes using Anchor’s program tools and frameworks. About You You’re a motivated practitioner who brings: A tertiary qualification in Social Work, Psychology, Youth Work or related discipline (final-year students may be considered). A solid understanding of the Children, Youth and Families Act 2005 and trauma-informed practice. Experience in case management within child, youth or family services. An ability to work with young people from diverse and complex backgrounds, including those in the statutory care system. Strong interpersonal, advocacy, and organisational skills.
