About the role We are seeking a casual Support Worker (Reablement) to join the Community Care team. In conjunction with the Client Reablement Officer, this position will plan, prepare, and provide appropriate services for the frail aged, people with dementia, and younger people with disabilities in accordance with the guidelines of the funding body and the City of Swan. As a Reablement Support Worke r, you will be required to work as part of a dynamic team, must have a positive outlook when providing support to the Members, is multi-skilled, able to identify changed responses in members and report concerns to the supervisor in a timely manner. You will also have a contemporary approach to supporting the Club philosophy. Key Accountabilities include, but are not limited to; Support the development and implementation of Business Unit Plan, projects and programs and the quality management principles. Interpret organisational policies and procedures to customers when required. Undertake cash management transactions according to current procedures Work collaboratively within a team environment to plan and implement programs for Community Care clients. Ensure that client meals are nutritious, well balanced, and cater for special dietary needs. Liaise with Transport Team to ensure the safety and maintenance of City of Swan vehicles. Encourage clients to maintain independence by using a Reablement, Person Centred Approach and Consumer Directed Care Approaches. Monitor and record daily, client’s mental and physical conditions and notify the Client Reablement Officer of any issues or concerns. Ensure safe, clean, and comfortable environment at all levels of service provision About you The successful candidate will have the following: Experience in manual handling of frail aged and people with disabilities and/or people with dementia Working knowledge of the ageing process and/or the effects of dementia Certificate III (or near completion) in service delivery for the aged, people with disabilities and/or people with dementia. Or equivalent experience (Desirable) National Police Clearance Check National Disability Screening check (NDIS) Driver’s licence First aid certificate For more information about the role please refer to the position description.
