About the Role The Clinical Lead / Care Partner – Support at Home is a senior, dual-function role combining care management, clinical oversight, and leadership within the Support at Home (SAH) program. Based from our Victor Harbor office, you will manage a portfolio of SAH participants while also providing clinical guidance and oversight for participants with complex needs, supporting other Care Partners and contributing to service quality, workforce capability, and clinical best practice. This role works collaboratively with participants, families, informal carers, internal teams, and external providers to ensure care is clinically appropriate, financially sustainable, and aligned with participant goals and funding. Key Responsibilities Care Management & Service Coordination Deliver person-centred care planning aligned to participant goals, preferences, and assessed needs Support participants to understand their approved services and funding, enabling informed decision-making Monitor, review, and adjust services and budgets in response to changing needs Maintain regular participant contact, including at least one care management activity per participant per month Conduct home visits for care planning, review, and complex care support Refer participants for reassessments and support plan reviews where required Ensure compliance with the Aged Care Quality Standards, particularly Standard 3 Clinical Leadership & Oversight Provide clinical oversight and support for participants with complex or high clinical needs Act as a clinical escalation point for Care Partners and service teams Support multidisciplinary care coordination, including assessment, prevention, referral, and health promotion Contribute to continuous improvement of clinical and care management practices Support workforce capability through coaching, guidance, and informal education Intake, Onboarding & Operations Coordinate intake and onboarding of new SAH clients Monitor waitlists and manage admissions efficiently Liaise with service providers to ensure timely, high-quality service delivery Generate purchase orders and manage participant budgets within approved funding Identify and escalate financial risks or discrepancies in collaboration with Finance and Program Support teams About You Registered Nurse with current AHPRA registration Experience in community, aged care, palliative, or complex care settings Strong understanding of person-centred care and aged care funding models Ability to provide clinical leadership and support within a multidisciplinary environment Excellent communication, coordination, and stakeholder engagement skills Ability to manage competing priorities while maintaining compliance and quality outcomes Current Driver’s Licence and willingness to travel Relevant clearances (Police Check, WWCC, Disability Employment Screening) Certificate IV in Training and Assessment (desirable, not essential) Why Join YNA & ASAH Competitive remuneration Senior, influential role combining clinical practice and leadership Supportive, values-driven organisations committed to quality aged care Training, professional development, and career progression opportunities Access to Employee Assistance Program (EAP) Inclusive and diverse workplace culture Opportunity to make a meaningful difference in the lives of older Australians How to Apply Submit your application via SEEK, including your resume and a cover letter outlining your suitability for the role.
