Carer Gateway is an initiative to provide carers with information, peer support, emergency respite, counselling or coaching. As our Case Manager , you'll connect carers to vital services, ensuring they feel supported, empowered, and connected in their important role. What you get to do: Connect with carers, assess their needs, and develop tailored plans that may include respite, counselling, and coaching. Collaborate with the team to create a safe space for carers to share their journey and access wraparound services. Work with medical and allied health professionals to create service plans that meet the carers' goals and preferences. What would help you succeed in the role: You will be experienced as a Social Worker, Case Worker, Case Manager or Support Coordinator with: A diploma or degree in aged care, community care, disability services or community services is highly advantageous. Demonstrated understanding of contemporary disability practice. Demonstrated understanding of the National Disability Insurance Scheme. Strong commitment to customer service and finding solutions to meet client needs. Excellent communication skills including cross cultural awareness. Understanding of the needs of diverse communities such as Aboriginal and Torres Strait Islander, culturally and linguistically diverse (CALD), and gay, lesbian, bisexual, transgender and intersex (LGBTI) communities. Demonstrated computer skills including Microsoft Office. Strong emotional intelligence and the ability to create an experience where a caller quickly feels heard and comfortable, often at a time when the caller may feel emotional, confused or frustrated navigating systems they may not understand in time of emotional stress.
