This managerial role offers the perfect opportunity for someone with a strong background in case management and home care to make a meaningful impact in the lives of our clients. Key Responsibilities: Manage a caseload of up to 75 clients, ensuring the highest standards of care. Conduct weekly client visits to assess needs and monitor care plans. Oversee the administration of home care packages, ensuring compliance and quality service delivery. Collaborate with care partners and support staff to develop tailored care plans. Implement best practices and policies to enhance client satisfaction and operational efficiency. Provide support and mentorship to team members in the field. Qualifications: Proven experience in case management or as a care partner within the home care industry. Strong knowledge of home care packages and relevant regulations. Previous aged care experience preferred. Excellent communication and interpersonal skills. Ability to work independently and as part of a team.
