Family Support Case Manager (Band 6) Full-time | 38 hours per week Contract: 16 February 2026 – 15 May 2026 Hours: Monday to Friday, 9.00am – 5.00pm We are seeking an experienced Family Support Case Manager to support vulnerable families with children and young people within a multidisciplinary service environment. This contract role focuses on child safety, wellbeing and family stability, with referrals primarily received through The Orange Door and Child Protection. Key Responsibilities Case Management & Service Delivery Accept and assess referrals from The Orange Door and Child Protection Manage a full caseload of vulnerable families Conduct comprehensive assessments, including risk and crisis assessments Develop, implement and review case plans in collaboration with families Ensure interventions align with the best interests of the child Provide practical, emotional and educational support to parents and carers Advocate on behalf of families with government and community services Support families to access appropriate programs, services and resources Group Programs Develop, facilitate and evaluate group programs for parents and children Deliver child-specific programs, including middle years groups Support children to build social, emotional and practical skills Promote connection to school and community Collaboration & Stakeholder Engagement Work as part of a multidisciplinary team Liaise with community organisations, service providers and statutory bodies Strengthen referral pathways for vulnerable families Use interpreters and culturally appropriate resources as required Documentation & Compliance Maintain accurate and timely case notes, assessments and reports Ensure all records are managed securely and confidentially Comply with child safety standards, privacy legislation and mandatory reporting obligations Work within relevant Victorian legislation and service frameworks Continuous Improvement Participate in service planning, evaluation and quality improvement activities Use client feedback to inform service delivery Engage in relevant professional development and networks Key Selection Criteria Qualifications & Experience Degree in Social Work, Psychology, Behavioural Science or a related field Demonstrated experience working with vulnerable families and children Strong knowledge of case management practices and risk assessment Experience delivering family and parenting interventions Experience developing and facilitating group programs Knowledge & Skills Sound understanding of child safety legislation and frameworks Ability to work effectively with culturally and linguistically diverse communities Strong interpersonal and written communication skills Ability to manage time, prioritise workload and work autonomously Intermediate computer skills, including Microsoft Office and electronic record systems Mandatory Requirements Current Working with Children Check Current National Police Check Current Victorian Driver’s Licence At Chandler Macleod, we are committed to fostering a diverse workforce where everyone is welcome.
