The Partner in the Community (PITC) Pathway Coordinator – First Nations supports people with disability across the consumer pathway, from referral through to discharge, including those transitioning out of or not eligible for the NDIS. As the first point of contact, the role provides timely advice, guidance, and practical support to individuals, families, and carers. Working in partnership with First Nations people with disability, their families, carers, and communities, this role builds strong, trust-based relationships and promotes culturally safe, inclusive, and community-led supports. The Pathway Coordinator collaborates across multidisciplinary teams to ensure smooth navigation of NDIS access, Early Childhood, and Local Area Coordination pathways, while maintaining high-quality service delivery and consumer satisfaction. This is an Identified Position, applicants must be of Aboriginal and/or Torres Strait Islander descent, as permitted under the Equal Opportunity Act 1984’ Key Responsibilities Act as the first point of contact, providing information, guidance, and support across the NDIS access and participant pathways Build and maintain strong, culturally safe relationships with First Nations people with disability, families, carers, and community stakeholders Support individuals to complete and submit documentation for NDIS access and referrals Provide cultural consultancy and advice to internal teams to promote inclusive and culturally appropriate practices Collaborate with community and mainstream service providers to strengthen inclusive service delivery Facilitate referrals to Local Area Coordinators (LAC) and Early Childhood teams and support smooth pathway transitions Develop Community Connections Plans and maintain community asset maps for individuals aged 9 years and over Utilise the Towards Inclusion Framework to build individual and family capacity, choice, and control Ensure timely provision of information and linkages to mainstream and community services Respond to enquiries via phone and email in a timely and professional manner Operate in accordance with relevant legislation, organisational policies, and workplace health and safety requirements Contribute positively to a collaborative, values-driven team environment About us We are proud to be part of the merged care organisation that will be known nationally as BaptistCare.
