The Client Cardinal Care Support Services is a disability support services provider that provides quality services in Darra, QLD. They are looking a full time Community Worker at Darra, QLD. The Role They are seeking a Community Worker who will work closely with individuals, families, and community groups to assess needs, create tailored support plans, and connect people with the resources and opportunities that will improve their quality of life. This is for a full time role with an annual salary of $77,000 to $90,000 depending on experience plus superannuation and will be based at Darra, QLD. Duties will include: Assess clients' needs and develop individualised support programs. Interview clients to understand their challenges and strengths. Plan, deliver, and evaluate community-based programs, workshops, and activities. Monitor client progress and prepare progress reports. Refer clients to external agencies for specialised assistance when required. Assess community needs and identify available resources in areas such as health, housing, employment, and training. Build partnerships with community groups, welfare agencies, government bodies, and local businesses to promote awareness of services. Support families and provide education and care for children, young people, and adults in community-based programs. Assist young people to address social, emotional, and financial challenges. Prepare reports, funding submissions, and documentation to support service delivery. Requirements Minimum Certificate IV in Community Services or related field Minimum 2 years relevant work experience as a Community Worker or in a similar community services role Minimum 2 years relevant work experience of case management, report preparation, and submission processes including NDIS documentation Minimum 2 years demonstrated experience working with vulnerable individuals, people with disabilities, or culturally diverse communities Minimum 2 years of experience dealing with mental and emotional disturbances and developmental disability Minimum 2 years of experience working with government agencies, non-profits, and other service providers Have a current or be willing to obtain a First Aid (HLTAID003 or HLTAID004) and CPR (HLTAID001) certificate Current Working with Children Check Current AFP Police Check Current NDIS Worker Screening Clearance Minimum two professional references from previous employers Excellent communication and interpersonal skills
