Limestone Coast Local Health Network, Country Health Connect, Mount Gambier Salary: RN/M3 $125,183 to $130,779 p.a. 12% Superannuation Leave Loading and Salary Sacrifice Benefits Permanent Full-Time position Eligibility Open to Everyone About the Role Limestone Coast Local Health Network is seeking an experienced and highly skilled Nurse/Midwife Consultant to join the Better Care in the Community (BCIC) program as a Care Coordination Facilitator. This senior clinical role provides expert nursing/midwifery leadership and care coordination for clients with chronic and complex conditions who are at risk of frequent or prolonged hospital admissions. Working with a high degree of autonomy, you will collaborate closely with hospitals, community health services, general practitioners, ambulance services and non-government organisations to improve hospital avoidance outcomes, enhance continuity of care and support people to remain safely in the community. Provide expert clinical nursing/midwifery care, case management and care coordination for clients with chronic disease and complex needs, with a strong focus on hospital avoidance. Lead and contribute to multidisciplinary care planning, complex discharge planning and service integration across acute, primary and community settings. Provide professional leadership, mentoring and clinical support to nursing, allied health and support staff within the BCIC program. Contribute to service development, quality improvement, education and research activities to improve client outcomes and system performance. Who are we looking for? A Registered Nurse or Midwife eligible for registration with the Nursing and Midwifery Board of Australia, with at least three years’ post-registration experience. A senior clinician with demonstrated expertise in chronic disease management, care coordination, hospital avoidance or community-based care. A confident leader with highly developed communication, problem-solving and change-management skills, able to work autonomously and collaboratively.
