You will be reporting to the NDIS Support Coordinator and accountable to the Community Health Programs Manager, your role plays a key part in coordinating Evidence, Access and Coordination and Planning (EACP) and Remote Community Connectors (RCC) programs across the Pilbara and Western Desert regions. Key Responsibilities Act as a central point of contact to support NDIS access, registration and planning for participants Coordinate EACP and RCC programs across remote communities Oversee recruitment, training and ongoing support of RCC staff Assist participants to identify and achieve personal goals using a person-centred approach Build and strengthen informal, mainstream and funded supports, including health, education, housing and justice sectors Liaise with families, carers, community groups and service providers Facilitate meetings, workshops and community presentations to raise awareness of the NDIS Contribute to the development of NDIS business cases in collaboration with management and consultants Complete accurate reporting in line with EACP/RCC contractual requirements Uphold organisational values, confidentiality, cultural safety, WHS and quality standards Location Information The Pilbara is known for its open spaces, striking natural environment and rich Aboriginal cultural heritage. Essential Selection Criteria Aboriginality, as provided under Section 50(d) of the Equal Opportunity Act 1984 (WA) Strong ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander peoples Tertiary qualification in Social Work, Psychology, Occupational Therapy and/or Certificate IV in Disability Services (or equivalent experience) Demonstrated knowledge and experience in NDIS Support Coordination Understanding of the National Standards for Disability Services Knowledge of co-morbidities impacting people in the Pilbara and Western Desert regions Understanding of the episodic nature of mental illness and its impact on daily functioning Knowledge of mainstream and community service providers, including disability and mental health services Your Employment in this role is subject to Police Clearance WA Working With Children Check NDIS Worker Screening Check Manual Drivers License Ability and willingness to drive 4WD vehicles To find out more about this position or to receive a copy of the job description please call (02) 9188 3989 or email caroline@curis.com.au and I will be in touch within 24 hours.
