About your opportunity: In this position you will The role is accountable at a high practice level to contribute to an interprofessional approach to the screening, assessment, care coordination and care planning of the elderly across all aspects of the service. Contribute to an innovative evidence-based model of health care for aged care clients with complex health care needs. Utilises evidence-based practice and professional judgement for a complex clinical caseload, to facilitate novel multidisciplinary clinical decisions and management with minimal clinical practice supervision. About your role: Provide quality Social Work specific clinical advice regarding patients to professional and operational supervisors, relevant service managers and other stakeholders regarding professional standards and clinical service development. Undertake quality improvement and evidence-based practice activities to ensure the provision of quality, cost-effective patient care. Deliver high level discipline specific clinical and consultancy services across all streams of the Older Persons Integrated Health Service. Recognised expertise, knowledge and skills are obtained through significant professional development activities, post-graduate education or formal qualification(s).
