About you: In your new role you will Provide care coordination of eligible home care clients by liaising with clients i.e. frail aged people and carers, health professionals and community agencies to ensure an effective, collaborative, and multidisciplinary approach to the planning and coordination of client care. Gather, analyse, and compile accurate data and statistics for reporting purposes for the National Data repository for the CHSP program to meet Minimum Data Set requirements. Manage the performance appraisal and development of staff.
