A Central QLD hospital requires a Specialist in General Practice from 04/03/2026 to 01/04/2026. Hospital in the Home (HITH) – 0.5 FTE This role supports the delivery of acute medical care outside the traditional hospital setting by overseeing patients admitted to the Hospital in the Home (HITH) program. Patients are clinically admitted but receive their treatment at home or in a community clinic. The aim is to reduce hospital admissions while maintaining high-quality, hospital-level care. Most home visits are carried out by HITH nurses, with the doctor primarily responsible for: • Providing clinical oversight and medical management for HITH patients. • Conducting initial assessments and reviews, either remotely or from the hospital or clinic base. • Collaborating with the multidisciplinary team to review treatment plans, monitor progress, and support discharge decisions. • Being available for consultation or escalation when patients under HITH care require medical input beyond nursing scope. This role is suited to a GP or SMO comfortable with managing acute presentations and working closely with nurses and allied health to coordinate community-based care. Geriatric Evaluation and Rapid Intervention (GERI) – 0.5 FTE This position focuses on improving emergency care outcomes for older adults through rapid geriatric assessment and early intervention. The GERI model works within the Emergency Department to reduce unnecessary admissions and support safe, efficient discharge for elderly patients. Roster details: • 10-hour shifts commencing at 7:30am • Includes visits to Residential Aged Care Facilities • May involve telehealth reviews for patients on the Capricorn Coast The doctor’s responsibilities include: • Providing senior medical support to the GERI nursing team. • Reviewing and managing elderly patients identified for geriatric intervention within the ED. • Supporting early decision-making, admission avoidance, and streamlined discharge planning. • Advocating for and embedding geriatric-focused pathways and nurse-led models of care. • Participating in quality improvement, education, and integration of the GERI model into ED operations.
