Our mission is to empower our participants through high-quality, person-centred care. The Role We are seeking a dedicated Community Worker to join our team in Everard Park. In this role, you will deliver structured, community-based support and coordinate practical pathways for clients who require assistance to participate, connect, and progress. This is a hands-on role requiring a blend of social support, advocacy, and administrative precision. Key Responsibilities In accordance with the professional standards for Community Workers, your duties will include: Assessing client needs and developing individualized support programs and plans. Conducting interviews to identify social, emotional, and financial challenges, strengths, and priorities. Planning, delivering, and evaluating community-based programs, workshops, and activities designed to foster independence. Monitoring client progress and preparing comprehensive progress reports and service documentation. Identifying the need for specialized assistance and referring clients to appropriate external agencies (health, housing, or employment). Locating local community resources and linking clients with essential health, training, and welfare supports. Building and maintaining professional relationships with community groups, welfare agencies, government bodies, and local businesses. Assisting young people and families to address social challenges through practical support and targeted community programs. What You Will Bring A strong ability to assess complex needs and translate them into actionable support plans. High-level interviewing and goal-setting skills with the ability to maintain professional boundaries. A disciplined approach to following procedures, maintaining records, and meeting NDIS Quality and Safeguards standards. Availability to work across a flexible roster, including weekends and public holidays, with travel between community locations. Minimum Requirements At least Certificate IV in Community Services (or a related discipline) is essential. Current Australian Driver’s Licence and reliable transport. Valid NDIS Worker Screening Clearance and Working with Children Check (WWCC). Valid working rights in Australia. Why Join Royal Care Australia?
