Your opportunity 12 month contract – Full time As our Care Coordinator, you will support older Australians to live independently by coordinating high-quality, person-centred home care services that align with their goals, wellbeing and preferences. Working collaboratively with clients, families, internal teams and community networks, you will ensure compliance, accurate documentation and continuous improvement in service delivery. With integrity and resilience, you will: Ensure services meet Aged Care Quality Standards, Charter of Rights and program guidelines (HCP, CHSP) Maintain timely and accurate client records, assessments and case notes in digital systems Build strong relationships with clients, families, carers and teams to support holistic care planning Capture feedback, escalate issues, respond to changing needs and contribute to quality initiatives What you’ll bring You are a compassionate, organised and proactive communicator with strong attention to detail and the ability to build rapport. You bring experience in community aged care coordination and a commitment to delivering exceptional client outcomes. You also share our organisational values of integrity, justice, compassion and excellence, embedding them in everything you do. You will also need: Certificate III or higher in Aged Care, Community Services or equivalent Previous experience in community-based aged care or home care coordination Strong communication and collaborative skills High level of organisation and initiative Intermediate computer literacy for electronic client records Ideally, you will also have: Knowledge of aged care funding - SAH (Support At Home) Experience contributing to continuous improvement in care services Why Anglicare?
