Why this role matters The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services. About the role As a Case Manager – Community Connections Program, this is your opportunity to provide case management support to people who are experiencing homelessness, are at risk of homelessness, or are living in low-cost or insecure accommodation, and who have multiple unmet health needs. Support will be delivered in a manner consistent with the Homelessness National Model of Care and the guidelines issued by the Department of Families, Fairness and Housing. Reporting to the Team Leader, this is a full-time role is contracted until June 2026, located in Nunawading, VIC. How you will make an impact Deliver CCP services in alignment with The Salvation Army’s National Homelessness Stream Model of Care, funding guidelines, accreditation standards and program specific frameworks. Provide comprehensive, client-centred case management including intake, assessment, case planning, regular reviews and case closure that addresses multiple and complex needs and supports sustainable outcomes. Deliver holistic, culturally safe and trauma-informed support that responds to individual circumstances, strengthens client capacity and promotes self-determination and active participation. Support clients to access, navigate and engage with a range of health, housing and community services through effective advocacy, coordination and partnership approaches. Maintain accurate records, complete reporting and administrative requirements, use systems and technology effectively and identify and manage risks in line with organisational policies and Workplace Health and Safety legislation. What you will bring Relevant tertiary qualifications in social work, human services, aged care, disability, community services, or related field (diploma level minimum), and/or relevant experience Demonstrated experience in working with vulnerable people A current VIC drivers licence and employee Working with Children’s Check What we offer The Salvos offer eligible employee's a well-balanced package of meaningful benefits including: Salary packaging up to $15,900 tax free $2,650 meal entertainment benefit.
