About the Role We are seeking a compassionate and dedicated Disability Service Officer to join our team in Bright Support, Shepparton. In this role, you will provide vital support, information and advocacy to people with disabilities, helping them achieve independence, wellbeing and meaningful participation in the community. You will work directly with clients, families, service providers and community organisations to coordinate supports and promote access to essential social, recreational, health and financial resources. In this role you will: Assist people with a disability, their families and carers to build and pursue their goals, exercise control and engage with the NDIS. Key Responsibilities Assess clients’ needs and plan, develop and implement tailored support, education and training programs. Conduct interviews with clients to understand the nature and extent of their challenges. Monitor, document and report on client progress to ensure effective service delivery. Refer clients to appropriate community, health, housing, welfare or financial services. Assess community needs and resources related to disability, health, welfare, housing and employment. Liaise with local community groups, government agencies, welfare organisations and private businesses to address community issues and promote available resources. Support families and provide assistance with education and care for clients in group homes, adult service units and community programs. Prepare reports, case notes, funding submissions and documentation for government bodies and service agencies. Required Skills and experience Minimum 1 year experience in similar role.
