We offer a vibrant, inclusive culture that values your expertise. About the role Join a dynamic multidisciplinary rehabilitation team where your senior expertise will have a genuine impact. As a Senior Social Worker , you will play a critical role in delivering high-quality, person-centred care, supporting clients and their families through complex rehabilitation journeys in a fast-paced private hospital environment. Working autonomously and using advanced clinical reasoning, you will lead discharge planning, coordinate referrals and provide supportive counselling to assist with adjustment, recovery and connection to community supports. You will also guide and mentor junior social workers and students, contributing to education, quality improvement and service development across the hospital, including the Day Program as required. Key responsibilities include: Deliver high-quality, evidence-based social work interventions using advanced clinical reasoning in a fast-paced rehabilitation environment. Conduct comprehensive assessments and collaboratively develop, implement and review rehabilitation goals with clients and families. Provide supportive counselling and psychosocial interventions to assist adjustment, recovery and wellbeing. Lead effective discharge planning, referral and liaison to ensure equitable access to appropriate community supports and services. Manage and prioritise a complex caseload to meet the needs of multiple clients fairly and within required timeframes. Maintain accurate, timely documentation and activity data in accordance with Royal Rehab policies and CMS requirements. Actively participate in multidisciplinary case conferences, goal-setting meetings and coordinated models of care. Provide supervision, education and professional support to social work students and junior staff, acting as a positive role model. Contribute to quality improvement initiatives, research, service development and strategic planning activities. Uphold the organisation’s model of care, values and legislative, policy and professional requirements.
