Where you'll be working Macquarie Hospital, North Ryde What You'll Be Doing The purpose of the NSLHD Health Contact Centre is to provide a safe, simple, reliable telephone – based ‘Single Point of Access’ to a range of Northern Sydney Local Health District (NSLHD) Health services for the community and service providers across the NSLHD. The NSLHD Health Contact Centre is responsible for the provision of community health intake and admission functions for a targeted range of community health services across the NLSHD, the facilitation of timely patient and service information across the care continuum to better inform decision making, service co-ordination and client/carer outcomes, reporting of service provider demand against service provider capacity and actively manage service wait lists where unavoidable, and the management of the at risk elderly and chronic care register. People of Aboriginal and/or Torres Strait Islander background are encouraged to apply. Candidates Will Need To Meet The Following Criteria What is your understanding of the Northern Sydney Local Health District, Acute/Sub Acute sector, Primary and Community Area and General Practice? Demonstrated knowledge and skills in delivering a customer focused service with excellent telephone manner.
