About The Bridge Centre The Bridge Centre is Australia’s first-of-its-kind Community Reintegration Centre, offering specialist post-release support. What You Will Do As a Nexus Case Manager, you will: Build strong, professional relationships with clients and their families to support reintegration Develop tailored Individual Reintegration and Support Plans pre-release Provide assertive outreach for up to three months post-release, ensuring clients engage with housing, employment, education, health, and community services Maintain and grow networks with state-wide service providers to maximise opportunities for clients Deliver crisis support and referrals when needed, ensuring no one falls through the cracks Contribute to program development, data management, and strategic growth This role is based across Ravenhall Correctional Centre (Ravenhall, VIC) and The Bridge Centre (Richmond, VIC), with outreach in the community and occasional interstate travel.
