You will lead and coordinate all activities related to client care. Conduct regular quality assurance visits (in person or via phone) to monitor care, address changing needs, and recommend additional services as required. Collaborate with clients, families, Caregivers, and other stakeholders to ensure care plans remain aligned with evolving needs and preferences. Maintain strong local knowledge and networks by staying informed about community services and engaging with health providers and referral sources to promote available support options. About you Tertiary qualification in a health-related field is desirable (e.g. social work, allied health, community services) Experience in Aged Care or Disability is required Strong organisational skills with the ability to manage time effectively, multitask, and adapt to shifting priorities under pressure. Excellent written and verbal communication skills, with accurate data entry and proficiency in Microsoft Office.
