The Community Care team is seeking a dedicated individual with a person-centred approach to monitor and manage client rosters, ensuring that continuity of care and client satisfaction are the priorities. You must be an individual with a proven background in disability and a solid understanding of the role of support workers and the needs of high-complexity clients. The successful applicant will be a team player, motivated, and can respond appropriately to ongoing changes. Your role will include: Assist clients in maintaining appropriate and effective rosters of support that align with their individual goals, preferences, and changing needs. Manage and maintain accurate client records, ensuring all information, documentation, and correspondence reflect current service delivery. Coordinate and facilitate meet and greets to uphold client choice and control, working collaboratively with support workers to ensure the right fit. Monitor support worker performance and client satisfaction, addressing concerns promptly to maintain high-quality service delivery. Oversee and maintain service templates to ensure consistency, compliance, and clear communication across client services.
