This role will include a focus on delivering interventions for patients with range of diseases, and a caseload focussing on strategies to maintain and improve health, functional ability and quality of life, and admission avoidance. The purpose of this role is to deliver Social Work clinical services as a part of a HP3 rotational arrangement to provide optimal, responsive, and flexible Social Work services in acute and community settings. Mandatory Requirements: All Social Work positions are required to provide 7-day availability for After Hours On-call rostering to respond to clinical emergencies for patients who meet specified clinical criteria at Ipswich Hospital.
