An exciting opportunity exists for a Service Manager Hospital Liaison Officers (Identified). The Aboriginal and Torres Strait Islander Health Directorate works across all facilities and provides high quality care and support to Aboriginal and Torres Strait Islander people. This is achieved with a multidisciplinary team approach to complex care for patients and their families, including inpatient support, case management, care coordination through the health service and a supported transition back into primary care. Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north. Key Responsibilities include: Demonstrate a high-level understanding of Aboriginal and Torres Strait Islander inequities, and health issues to deliver informed, evidence based culturally appropriate care including knowledge of relevant legislation, standards, policies, and procedures Provide high level leadership and management to support the delivery, co-ordination, development, and monitoring of the Aboriginal and Torres Strait Islander health equity strategy, workforce action plan and other strategies and services related to Aboriginal and Torres Strait Islander Health. Remuneration value up to $148 323 p.a., comprising salary between $121 360 - $129 998 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (AO6).
