We are currently seeking a motivated and compassionate Social worker to join our multidisciplinary team at Seymour Health. The position includes supporting Ambulatory Care, Acute Ward, Hdx, HARP (Hospital Admission Rick Program and Barabill House Aged Care Facility. This position is a permanant 0.5 EFT position (rostered days flexible). For further information on the role, please refer to the Position Description attached. Ideally you will possess: Ability to independently provide clinical reasoning and clinical competencies, including expertise in the use of relevant assessment tools, treatment methods, and case management skills Experience working with people experiencing complex needs, health conditions, including trauma, substance use, or family violence. Experience in person centered care and advocacy. Experience with discharge planning and service availability Knowledge around Aged Care process and NDIS.
