About your opportunity: Provide an opportunity for young people in secondary school to access health information, advice, support, brief intervention, and referral through confidential individual health consultations. Provide leadership to effectively plan, manage and evaluate care activities at an advanced practitioner level within the primary health care area. Facilitate the development of partnerships and work collaboratively with young people, their families, the school community, other services and agencies to plan, implement and evaluate health promotion programs and activities, to address youth health issues and to improve access to services. Effectively communicate at all levels across the organisation to promote effective team functioning. Support schools to use a whole of school approach to health promotion (Health Promoting Schools framework) to encourage a coordinated and comprehensive approach to the planning and implementation of school policies, programs, activities and environments.
