About the School Hester Hornbrook Academy is an independent, Special Assistance School delivering flexible, inclusive education for young people aged 15–25 across Greater Melbourne. Guided by our trauma-informed HOPE (Healing Oriented Program of Education) framework, we offer integrated academic and wellbeing support through applied learning pathways including VPC, VM, and VETiS. Position Purpose The Pathway and Transition Co-ordinator supports students and alumni to achieve positive post-school outcomes through transition planning, alumni engagement, and tailored support. As part of the CAPE Team, this role coordinates the Alumni Program, manages School-Based Apprenticeships and Traineeships, and provides ongoing outreach and case management to former students. The position ensures every student has access to meaningful pathways and is tracked for five years after leaving the academy, using these insights to strengthen future programs. Working collaboratively with staff, families, and community partners, the Pathway and Transition Co-ordinator plays a vital role in empowering young people to succeed beyond school.
