About the School Hester Hornbrook Academy is an independent, Special Assistance School delivering flexible, inclusive education for young people aged 15–25 across Greater Melbourne. Position Purpose The Career Transition Co-ordinator plays a pivotal role in supporting students as they prepare for life beyond school. Working within the CAPE Team, this role helps students identify, plan, and achieve positive post-school pathways through education, training, or employment. The position provides individual and group support, facilitates transition programs, and works closely with the Alumni and Pathway teams to ensure every student experiences a smooth, supported move into the next stage of their journey. This role combines outreach, mentoring, and coordination to help young people build confidence, connection, and direction as they transition from school to independence. Location: This position is currently available for application for the following campuses: - Werribee - Sunshine - City - South Melbourne/Hybrid Key Selection Criteria (abridged; full details in Candidate Information Pack) We are seeking a motivated and experienced Careers Co-ordinator to lead our school-wide careers and pathways program. You will bring: Qualification in Youth Work, Social Work, or Career Development (or equivalent). Experience supporting young people to explore education, training, and employment pathways. Knowledge of tertiary, TAFE, vocational, and employment systems. Experience facilitating Career Action Plans, Morrisby profiling, and transition readiness programs. Strong ability to work collaboratively within a multidisciplinary team. Excellent communication, advocacy, and relationship-building skills. Proficiency in Microsoft 365, Teams, Canvas, and other digital systems for documentation and reporting. Understanding of trauma-informed and inclusive practices aligned with the HOPE model.
